Day-Off Camp Cancellations

This form is for those enrolled in Day-Off Camps at Fitness Alive. Complete the form to cancel a date for your camp. You will get a confirmation email when your lesson is canceled.

Here is Fitness Alive’s Day-Off Camp cancellation policy:

Early Cancel with at least 2 weeks notice ahead of camp date: Full refund

Late Cancel with less than 2 weeks notice ahead of camp date: No refund
No-show: No refund

Sick policy: If your child is sick, please reach out to us so we can make an accommodation for an account credit to use for a future service. We may ask for a doctor's note.
